Dental, Orthodontic & Optical Benefits

For your convenience, Request for Dental Benefits and Request for Optical Benefits forms are now available online.

At Home Program benefits include dental orthodontic and optical expenses directly related to the child's disability, if the need is not met through another program or insurance plan.

  • Dental: maximum $700 per year
  • Orthodontic: maximum $5,000 lifetime
  • Optical: prescription lenses and frames to a maximum of $150 per year

Note: The program does not cover regular dental care (checkups and cleaning).

Benefits must be pre-approved.

To apply for Dental or Optical benefits, please print a Request for Dental Benefits or a Request for Optical Benefits form.

The Healthy Kids Program provides basic optical and dental benefits for families who qualify for Medical Services Plan premium assistance. Some children are eligible for both the Healthy Kids Program and the At Home Program.
