Justification for Equipment and Supplies
Equipment and supplies necessary for the child's intervention must be requested using a Justification for Equipment and Supplies form completed by a professional/specialist from the following list:
- Behaviour Consultant
- Physiotherapist
- Speech Language Pathologist
- Occupational Therapist
- Registered Psychologist
- Registered Psychologist Associate
- Psychiatrist
- Neurologist
- Paediatrician

Effective immediately, the following policy applies to requests for Weighted Blankets: All Justification for Equipment and Supplies forms must be completed by a Registered Occupational Therapist or Registered Physical Therapist.
MCFD has received information from the Autism Society of Canada regarding the use of weighted blankets with children. Please read the attached Advisory regarding their use.
