Case Manager
A Case Manager is required for all youth being referred to the Maples. A Case Manager may be a professional who has an ongoing relationship with the family such as a mental health clinician, a probation officer, or a social worker. Psychiatrists and medical doctors are not generally considered Case Managers.
The role of the Case Manager is as follows:
- To educate family members about the Maple's program to which the youth is being referred and to secure their consent for the referral;
- To ensure the youth and their caregiver has a clear understanding of program expectations, expected outcomes and possible risks;
- To help the youth and/or their caregivers talk about any concerns or anxieties they may have about the referral or while receiving services;
- To attend the admission, Care Plan development, and integration meetings if the youth is referred to the Response Program. The latter meeting usually occurs approximately two to three weeks after the Care Plan meeting;
- If the youth is referred to a treatment program, to attend the admission, mid-case, and discharge meetings (additional meetings may be required);
- To coordinate other community players and resources in collaborating on providing or developing services as recommended in the youth's Care Plan;
- To assist in integration of youth into their community after they have left the Maples.
