The Youth Transition Program (YTP), established in 2005, is a resource program for deaf, hard of hearing, and deaf blind youth. Together with families, communities and service providers, YTP endeavours to support them in planning for their futures.

The transition years, from adolescence to early adulthood, present a critical developmental period for these youths. YTP offers a unique and vital link toward building a network of support and creating a path toward adulthood.

Services include, but are not limited to, development of an Individual Transition Plan (ITP), family/community planning meetings, appointment support, assessment of strengths and interests, and connection to community resources. YTP works with existing agencies to help consolidate a coherent community-based transition plan for youth.

It is not the intention of YTP to provide services that are already mandated by other ministries or service providers, rather YTP seeks to fill service gaps for this unique population, and to facilitate links for youth within their communities.

Eligibility

Deaf, hard of hearing, and deaf blind youth, who are residents of British Columbia, are between the ages of 15 to 21, and use sign language for all or part of their communication are eligible for YTP services.
