Transition Planning

There are two key transitions in the lives of children and youth: the pre-school to school phase and the high school to adulthood phase. Effective transition planning can help to ensure that these transitions go smoothly and successfully.

Transition planning is a partnership involving the individual with special needs, their family, local service providers, school personnel and other key people involved with the child or youth.
